Why is rotating injection sites hard to do well?
Instructions for many subcutaneous injectables advise varying the injection site from dose to dose, commonly to reduce skin irritation. Following that on a weekly cadence means remembering where a shot went one or two weeks ago, which is exactly the kind of detail that slips. Over time, a usual spot can turn into the only spot.

The whole log is date, dose, and site. It works when the site gets noted on every dose, including the rushed ones, and when the last few entries can be read back in seconds. If a spot ever needs a clinician's eyes, the log can say when it was used and how often.
